Special Dietary Requests
If your student has a food preference or a medically diagnosed allergy please contact your student's school nurse.
For a personal food preference (e.g. vegetarian) please complete a Dietary Preference Form.
For a medically diagnosed food allergy and/or intolerance, a Medical Statement Form or Medication Order completed by a licensed medical professional will be required as part of your student's health record.
To cancel or stop an existing special diet request, a Special Diet Discontinuation Form must be completed by a licensed medical professional.

Allergy Menu Information
The Nutrition Services Department allows the public to view which menu items contain the 9 Major Food Allergens (milk, eggs, fish, shellfish, tree nuts, peanuts, wheat, soybeans, sesame).
To view the allergen information go to MealViewer Schools and select a school. Select a specific day on the calendar menu and on the left hand side of the page click on 'Allergens'. Select which allergen you would like to view. The menu items that contain the selected allergens will be crossed out with a red line.

Non-Major Food Allergens
D51 has received requests for ingredient lists to ascertain which menu items contain dyes, annatto, caramel color, paprika, corn syrup and others. These types of food ingredients are not currently identified by the FDA (Food & Drug Administration) as Major Food Allergens.
D51 and menu software providers will not list or identify the above items due to frequent product manufacturer and formulation changes. D51 is only able to provide menu software which readily identifies the 9 major food allergens listed above.
D51 will create a special menu for non-FDA Major Food Allergens if the student has a Medical Statement Form signed by a licensed medical professional.
Disclaimer: Mesa County Valley School District 51 will make every attempt to identify allergens in our ingredients that may cause allergic reactions for those with food allergies and present those allergens through the official menu pages on the school district website. We provide annual training for all food production staff on the severity of food allergies. Please be advised menu items may contain allergens, may come in contact with items containing allergens, and there is always a risk of contamination or cross-contamination. In addition, some students in the cafeteria are bringing lunches from home. There is also a possibility, food manufacturers could change the product formulation or substitute a food item at any time, without notice. Students, parents, and staff concerned with food allergies need to be aware of these risks and Mesa County Valley School District 51 will not assume any liability for adverse reactions to foods consumed or items a student may come in contact with while eating in the school dining areas.
